Does it cost anything to take part?
No. Study visits, study procedures, and the investigational product are provided at no cost to participants.
Do I need a referral from my doctor?
No. You can send this form yourself. With your permission we can keep your regular doctor informed, and your care with them continues either way.
What happens to my information?
It is used to check you against studies that are enrolling, and it is kept confidential. It is not sold or given to unrelated third parties.
